CME Credit For The Upcoming Family Practice Review & Reunion
AOA - Grandview Medical Center is accredited by the American Osteopathic Association (AOA). The AOA designates this program as meeting the criteria for 25 credits in category 1-A. Determination of credit is pending. CME Certificate of Hours will be provided in your registration packet. Registrants must sign this form attesting to the total hours attended and return the form to the registration desk to be verified to receive credit. The verified copy of this form should be saved for your records. Only those physicians who return this form will be given CME credit for this program. Physicians who fail to return this form will not be given any CME credits as the FPRR certifies CME directly to the AOA.
State Medical Board of Ohio - Allopathic physicians may claim Category 1 credit for attending this conference. Application for CME credit has been filed with the American Academy of Family Physicians (AAFP). Determination of credit is pending.
For purposes of re-certification through ANCC, offering will be accepted as part of the 50% requirement provided by organizations accredited through the Accreditation Council on Continuing Medical Education - Category 1.
